The NHS Health Check Clinical Development Nurse will provide clinical leadership and oversight for the delivery of the NHS Health Check Programme across Leeds. The role will support the high-quality and consistent delivery of the programme, ensuring that it aligns with national guidance and local population health priorities. The requirement for this role is 7.5 hours per week.

Main duties of the job

  • Act as a clinical advocate for the NHS Health Check Programme, supporting GP practices to improve the uptake, quality, and outcomes of NHS Health Checks.
  • Work in close partnership with the Deputy Service Delivery and Relationship Manager, the Operational Service Delivery & Performance Lead, Public Health, Leeds City Council, and General Practice teams.
  • Provide clinical leadership and specialist expertise to support the delivery, development, and continuous improvement of the NHS Health Check programme across Leeds.
  • Promote best practice in cardiovascular disease prevention, early detection, and risk management in line with national guidance, NICE recommendations, and Public Health priorities from Leeds City Council.
  • Support clinicians and healthcare professionals in delivering high-quality NHS Health Checks through the implementation and ongoing development of the quality assurance framework.
  • Act as a clinical champion for preventative health, advocating for the value and impact of NHS Health Checks.
  • Support the development, implementation, and evaluation of NHS Health Check initiatives, pathways, and service improvements.
  • Promote and support quality improvement initiatives aimed at increasing NHS Health Check uptake and improving clinical outcomes.
  • Review performance and population health data to identify trends, inequalities, and variation in delivery.
  • Provide clinical insight and practical support to the operational team and general practice teams.
  • Work with GP practices and PCNs to review and strengthen Standard Operating Procedures (SOPs) relating to NHS Health Check delivery.
  • Ensure the NHS Health Check programme is delivered in line with national guidance, clinical standards, and local policies.
  • Contribute to internal clinical governance processes, including risk management and quality assurance audits.
  • Deliver NHS Health Check training and contribute to the development of healthcare professionals.
  • Promote awareness and understanding of the NHS Health Check programme across primary care.
  • Support integration between NHS Health Checks and wider prevention programmes.
  • Champion a population health and health inequalities approach.
  • Engage with a wide range of stakeholders across primary care, Public Health, Leeds City Council, and community organisations.
  • Apply innovative, creative and flexible thinking to problem solving.

Person Specification

  • Experience delivering the NHS Health Check programme.
  • Experience presenting information or facilitating training, workshops, or stakeholder engagement sessions.
  • Experience working within primary care, community health, or public health settings.
  • Experience supporting or delivering preventative health initiatives.
  • Experience using clinical judgement and evidence-based practice.
  • Experience working collaboratively with multidisciplinary teams.
  • Experience interpreting and using performance or population health data.
  • Experience contributing to clinical governance activities.
  • Registered Nurse with current NMC registration.
  • Degree in Nursing, Public Health, or a related health discipline.
  • Demonstrable experience of working to reduce health inequalities.

Qualifications

  • Evidence of continuing professional development (CPD) relevant to cardiovascular disease prevention.
  • Training, leadership or coaching qualification.
