We are seeking motivated and patient-focused GPs to join our Community Beds Service. This role involves delivering high-quality, patient-centred care within a multidisciplinary team across community bed bases in Leeds. The post-holder will provide direct clinical care to patients who are typically transitioning from acute hospital settings or admitted from the community, supporting recovery, rehabilitation, and ongoing care needs. The role requires close collaboration with a wide multidisciplinary team and a strong focus on safe, effective, and coordinated care delivery.

Main duties of the job

  • Clinical Care & Ward-Based Delivery: Undertake regular ward rounds across community bed bases, providing comprehensive medical reviews of patients. Assess, diagnose, and manage a wide range of acute and chronic conditions, with a particular focus on frailty and complex comorbidity. Develop and review personalised care plans in collaboration with the multidisciplinary team. Support proactive clinical decision-making to optimise patient outcomes and experience.
  • Advanced Care Planning & Patient-Centred Care: Lead and support advanced care planning discussions, including ceilings of care and end-of-life decision-making where appropriate. Communicate effectively and compassionately with patients and their families/carers regarding diagnosis, prognosis, and care plans. Ensure care is aligned with patient preferences, best interests, and legal/ethical frameworks.
  • Multidisciplinary Team Working: Work closely with nursing, therapy, pharmacy, and medical colleagues to deliver coordinated, holistic care. Actively participate in MDT meetings, case discussions, and care planning.
