We are seeking an experienced and motivated clinical pharmacist to join our new community care bed team. This is a key role responsible for reviewing patients' medicines use within the four community care bed bases across Leeds, working with a wide multidisciplinary team including consultant geriatricians, GPs, advanced care practitioners, pharmacy technicians and non-registered carers, in an innovative partnership between Leeds GP Confederation, Leeds Community Healthcare NHS Trust and Leeds City Council.
The post-holder will provide clinical knowledge to ensure prescribing and use of medicines in community beds is safe, effective, and high quality, and support pharmacy technicians who are employed through partner organisations. The role will be deployed in matrix working across the four community bed bases, working across organisational boundaries to give safe and effective care to patients who need additional care after discharge from hospital before returning home.
Main duties of the job
- Key Responsibilities:
- Clinical medication review: Provide specialised pharmaceutical advice to health care professionals, non-registered caring staff, patients and carers.
- Participate in the community care bed MDT providing medicines related advice and targeted clinical medication reviews to community care bed patients identified as high risk or need, working within a team of senior and training doctors, advanced care practitioners, pharmacy technicians and carers.
- Support for community care bed bases: Advise, support and work directly with community care bed staff to identify any problems with systems, policies and procedures that support the medication needs of the patient including the safe prescribing, handling and administration of medicines.
- Answer relevant medicine-related enquiries from community care bed staff.
