The predicted salary is between 30000 - 40000 £ per year.

This is a fantastic opportunity to join our newly formed Weight Management Interface Hub Team. Working within the Service Delivery Team as a Healthy Weight Advisor within the Weight Management Interface Hub, you will support adults and families who are overweight or obese by providing support, guidance and signposting to other relevant services. Drawing on your experience of helping individuals with lifestyle modification and signposting/social prescribing, you will contribute to the overall delivery of the Weight Management Interface Hub service. You will work closely with and alongside a team of Health Coaches and registered healthcare professionals; including nurses, dieticians and physiotherapists.

You will be passionate about working with individuals to begin the life-changing processes necessary for permanent and continued weight loss and weight maintenance. You will understand challenges and barriers that adults face with relation to their health (both physical and mental) and wellbeing. You will engage with and build a good rapport with adults from varying communities across Leeds. You will utilise your existing knowledge of weight management approaches to offer non-clinical wellbeing advice and signposting to other relevant services and resources.

The ideal candidate will come from a background in a public facing role, be confident in conversing with people from varied backgrounds and have a detailed understanding of weight management provision.

Main duties of the job
  • Being part of a wider team that will deliver our adult weight management support to adult patients
  • Deliver evidence-based education and signposting relating to weight management
  • Support people with a person centred approach
  • Working with individuals using SMART goal setting
  • Keep detailed consultation notes, recording data accurately, ensuring all forms and other documentation is completed correctly
  • Be able to organise own workload in order of priority and deadlines, daily administration tasks and other work-related activities
  • Undertake and complete all mandatory and additional skills training as required
  • Work with service users and internal/external stakeholders to co-produce meaningful session materials
  • Contribute to the contract performance by submitting regular case studies to the line manager
  • Contribute to training and support new staff

Job responsibilities
  • Significant experience of supporting people make positive lifestyle change
  • Significant experience in working with service users with needs arising from being overweight and obesity

Person SpecificationExperience
  • Experience of working in a weight management / lifestyle modification service
  • Experience and ability to demonstrate coping with change
  • Experience of working successfully with a range of stakeholders both internal and external to the organisation
  • Experience of working in an NHS or Local Authority organisation
  • Excellent interpersonal skills with the ability to influence and negotiate
  • Experience of working with members of the general public
  • Experience of working with people from seldom heard or disadvantaged population groups
  • Demonstrates good self-awareness, regularly reflecting on personal performance to support continuous growth
  • Working on own initiative to deliver to deadlines
  • IT literacy including word processing, power point and database with the ability to produce power point presentations as well as reports to stakeholder boards
  • Maintain confidentiality when required
  • Excellent problem-solving skills
  • Experience of using SystmOne (training will be provided if required)
Qualifications
  • Evidence of ongoing professional development in field of expertise (weight management, lifestyle modification, general health, social prescribing)
  • Awareness and understanding of key challenges for people that are overweight or obese and strategies to address these
  • In-depth knowledge of evidence-based approaches to supporting people with positive lifestyle modification
  • Recent experience in a role where you have directly provided weight management support
