An exciting opportunity has arisen within the Integrated Care Delivery Service, and we are recruiting an Admission & Discharge Coordinator to work alongside our team at Dolphin Manor Care Home in Rothwell.

We are looking to recruit a well motivated and dynamic individual to work as part of the team at Dolphin Manor Specialist Short-Term Dementia Service. You will take responsibility for the coordination of all admissions and discharges, ensuring prompt action is taken to address any factors which could cause a delay in a person’s discharge home. You will have excellent communication skills and be able to successfully represent our priority to reduce unnecessary length of stay and prevent delayed discharges.

The successful candidate will demonstrate skills, knowledge and experience in the following key areas of responsibility:

  • Support the safe admission processes, including responsibility for medicine reconciliation and transcription.
  • Highlight as early as possible people who are ready for discharge and work with the MDT to coordinate and facilitate their onward move.
  • Be an advocate of strengths-based social care and for promotion of approaches to maximise independent living and championing the city-wide Home First ambition.
  • Coordinate referrals to system partners and external organisations such as Homecare Providers, Care Homes and Neighbourhood NHS teams and Active Recovery.
  • Update the Multi-Disciplinary Team on the progress of patient discharges and any delegated actions.

Dolphin Manor is delivered in partnership with NHS organisations in Leeds, providing specialist short-term 24-hour care to people with Dementia. The service focusses on recovery of wellbeing for people experiencing psychological distress and behavioural needs.

This role is based in the UK. The post holder will work within the Recovery Hub and support the multi-disciplinary team. The post holder will coordinate the safe and timely admission and discharge of customers to and from the Recovery Hub, ensuring that the person and their carers are provided with a high quality experience from admission through to discharge.

The post holder will contribute to the reduction in delayed transfers of care and a reduced length of stay, supporting the wider Leeds health and social care systems.

Responsibilities include:

  • Coordinate with system partners and external organisations to agree timely admission of customers from both community and acute settings.
  • Gather basic discharge information on admission and start discharge planning from the earliest opportunity.
  • Support the safe admission processes, specifically with responsibility for medicine reconciliation and transcription.
  • Highlight as early as possible customers who have no reason to reside within the Recovery Hub and are ready for discharge, ensuring they are discharged in a safe timely manner.
  • Advocate of strengths-based social care and for promotion of approaches to maximise independent living and champion the city-wide Home First ambition.
  • Coordinate referrals to system partners and external organisations such as Homecare Providers, Care Homes and Neighbourhood NHS teams and SkILS Reablement service.
  • Contact system partners such as GP’s, Community Services and Care Homes to request/provide additional information and/or inform them of discharge planning.
  • Attend and action appropriate tasks as delegated by the Registered Manager or Deputy Manager following Multi-Disciplinary Team meetings.
  • Update the Multi-Disciplinary Team on the progress of patient discharges and any delegated actions.
  • Ensure ‘No Reason to Reside’ data is reordered, maintained, updated.
  • Escalate issues that cannot be resolved within agreed timeframes.
  • Attend family meetings and clinical meetings (e.g. MDT, Best Interest) and provide feedback post-meeting to the Registered Manager or Deputy Manager and appropriate persons involved in the customer’s care as necessary.
  • Liaise with the team on a daily basis for an updated position on customer progress.
  • Communicate in an appropriate and effective manner to customers, families or carers where there may be barriers to communication such as language barriers, hearing impairments or cognition issues.
  • Update customer records as appropriate, following the Council’s policies and procedures.
  • Maintain personal competency in the use of electronic care records such as CIS and PPM+.
  • Comply with the requirements of all Leeds City Council policies.
