An exciting and unique opportunity has arisen for an experienced mental health nurse to be part of the complex psychosis pilot operating between The Assertive Outreach Team and West and North West CMHT. There are 2 parts to this position: initially a 12-month Assertive Outreach Team pilot role clinically based in a CMHT, then a substantive post as a Mental Health Nurse Case Manager within The Assertive Outreach Team.

Main duties of the job:

  • As part of the pilot, you will provide early help and engagement interventions to service users with assertive engagement needs that do not currently meet the threshold for the Assertive Outreach core offer across the West and North West CMHT/Hub.
  • The pilot role works clinically within the CMHT, with service users presenting with complex psychosis, to provide assertive interventions aimed at re-establishing connections with the CMHT.
  • The pilot role will maintain close working relationships with AOT and will work in a way to promote positive partnerships and support with transitions between AOT and the CMHT.
  • At the end of the Pilot phase, you will return to The Assertive Outreach Team as a Case Manager.
  • As a case manager in AOT you will offer individual packages of care working with a smaller caseload than is typical in a CMHT of twelve service users as a full time worker within a team approach structure.
  • This care will be delivered through the team approach in a multidisciplinary team, therefore good communication and team working skills are necessary.
  • The MDT team will support you to deliver quality care to service users both in the community informally, subject to CTOs, and service users who require hospital admission.

Detailed job description and main responsibilities:

  • The work in the pilot phase will require a flexible and assertive approach to connect with these service users, as well as developing positive working relationships within the CMHT and AOT.
  • The interventions will be flexible and person centered, with the emphasis on supporting complexities and re-engaging with mental health services.
  • The pilot also has a function of supporting transitions and communication between AOT and the CMHT.
  • This includes acting in a bridging role between both teams to support referrals and transitions, as well as providing advice and consultation around complex psychosis within the CMHT.
  • The pilot role will include attending regular MDT and team meetings with AOT as well as the clinical work within the CMHT.
  • You will be involved in the evaluation of the pilot role along with the Occupational Therapy Rehab Lead for Complex Psychosis.

Case Manager in The Assertive Outreach Team:

  • To work as a member of the Assertive Outreach team in providing a city wide service for patients with highly complex, severe and persistent mental health problems seven days per week;
  • To provide day to day co-ordination of the team, as required;
  • To act as a case manager, responsible for the management of a case load including the assessment of care needs, the development, implementation and evaluation of care packages;
  • To ensure an effective learning environment for students and other staff, regardless of the professional background;
  • To carry continuing responsibility for the setting and monitoring of standards of care in line with Clinical Governance requirements;
  • To promote nursing values and interventions in the MDT and with service users;
  • To establish and maintain good working relationships with other community teams/agencies within the statutory voluntary and private sector.

Person specification:

Qualifications:
  • Have a nursing (mental health) qualification and registration with the NMC
Experience:
  • Significant post registration experience of working with service users who have mental health difficulties who experience psychosis
Skills:
  • Car Driver - Community post requiring a car driver with a valid driving licence
