Are you an experienced Occupational Therapist with a passion for enabling safe, person-centred care while promoting independence?

We are seeking a confident and resilient OT to support proportionate care reviews across home-care packages. You will use your moving and handling expertise, equipment knowledge, and professional judgement to identify where single-handed care can be introduced safely and appropriately—reducing unnecessary double-handed care without compromising quality, dignity, or outcomes.

The role

You will work flexibly across a hybrid model, completing assessments, collaborating with care providers and families, and delivering practical recommendations that support safer, more independent daily living.

A key part of the role is building trust with stakeholders: clearly explaining how equipment, adapted techniques, and effective moving and handling plans can make one-carer support safe and sustainable.

Key responsibilities

  • Conduct holistic, person-centred assessments of home-care packages.
  • Review double-handed care arrangements and identify opportunities for safe single-handed care.
  • Recommend and assess suitable equipment, including hoists, slings, transfer aids, and other assistive technology.
  • Develop practical moving and handling plans and support their implementation.
  • Work closely with service users, families, care agencies, and multidisciplinary colleagues.
  • Provide clear professional rationale and reassurance when proposing reductions in care provision.
  • Ensure all recommendations prioritise safety, choice, dignity, independence, and positive risk-taking.
  • Monitor outcomes and ensure changes are only made when clinically appropriate and safe.

About you

You will be an experienced Occupational Therapist with a strong background in moving and handling, community practice, and equipment provision. You will be comfortable managing sensitive conversations and professionally challenging assumptions where a package of care may be more than is required.

We are looking for someone with:

  • Occupational Therapy qualification and HCPC registration.
  • Strong practical knowledge of moving and handling techniques, risk assessment, equipment, hoisting, and sling selection.
  • Experience reviewing care packages, ideally including proportionate care or single-handed care assessments.
  • The confidence to influence care providers and partner organisations through sound clinical reasoning.
  • Excellent communication, negotiation, and report-writing skills.
  • A resilient, professional, and compassionate approach.
  • A genuine commitment to person-centred practice—recognising that reductions should onlybe made where they are demonstrably safe and in the individual’s best interests.
