Your responsibilities will include: Completing Care Act assessments, reviews and support plans Managing complex caseloads within the community Undertaking safeguarding enquiries and risk assessments Working closely with families, health professionals, providers and partner agencies Supporting adults to access the right care, services and resources Promoting independence, choice and positive outcomes What you’ll need A recognised Social Work qualification Active Social Work England registration Experience in Adult Social Care, preferably within a locality or community team Confidence managing a busy caseload and undertaking community visits Strong assessment, safeguarding and care-planning skills Interested?
