You will manage your own caseload of adults with complex healthcare needs, delivering person-centred care with clinical autonomy in a hybrid role, Monday to Friday 9am-5pm, with no weekends or nights.
You will guide frontline teams, provide clinical supervision, and work with families and external services to achieve excellent outcomes.
