  • Act as a key point of support for less experienced staff, offering guidance around decision‑making, prioritisation and best practice.
  • Work within a light‑touch triage system, ensuring cases are appropriately managed and workflows remain efficient and person‑centred.
  • Mentor others, maintain high standards, and be a reliable source of knowledge within a fast‑paced environment.
  • Work closely with a multidisciplinary network, contributing to effective discharge planning and ensuring individuals are supported to return home safely with the right care in place.
