The Role
You'll work independently while being fully supported by an experienced multidisciplinary team, delivering high-quality nursing care to adults within their own homes. You’ll be responsible for:
- Completing holistic clinical assessments and developing personalised care plans
- Delivering safe, evidence-based nursing care and treatment
- Monitoring patients, evaluating outcomes and responding to changing clinical needs
- Educating and supporting patients, families and carers
- Working collaboratively with GPs, community services and multidisciplinary teams
- Maintaining accurate clinical documentation and ensuring the highest standards of patient safety and clinical governance
