Hybrid working is available, up to two days per week from home. This permanent role involves rapid onboarding and support to get you into the position quickly.
Key Responsibilities
- Oversee a busy service supporting safe and timely discharges across a 200-bed pathway.
- Allocate cases, monitor performance, and ensure assessments and reviews are completed within expected timeframes, including 14–28 day assessments and 6–12 week reviews.
- Guide and support the team through a light-touch triage system, ensuring efficient workflows and person‑centred outcomes.
- Maintain high standards of practice while supporting staff with decision‑making, prioritisation, and complex cases.
- Collaborate with a multidisciplinary network to drive effective discharge planning and support individuals to return home safely.
Qualifications & Requirements
As this is a statutory role, you must be a qualified Social Worker registered with Social Work England and have experience in supervising or managing within adult social care.
