We’re currently recruiting for an exciting opportunity within a busy Transfer of Care / Hospital Discharge Team, where your work will have an immediate impact on helping people return home safely and regain their independence.

About the Role
Based within a fast-paced hospital environment, you’ll be supporting safe and effective discharges across a 200-bed pathway. You’ll work with individuals to help them return home with appropriate care packages, while ensuring further support is available where required through re-referrals.
Your responsibilities will include:
- Completing assessments within a 14–28 day timeframe
- Carrying out 6–12 week reviews to monitor progress and outcomes
- Supporting a light-touch triage approach to ensure individuals receive the right level of intervention
- Working collaboratively with health professionals and partner agencies to deliver effective discharge planning
- Promoting independence and reducing avoidable hospital readmissions
You’ll become a key part of a supportive, multi‑disciplinary team where collaboration, communication, and person‑centred practice are at the heart of the service.
