Leaders in Care is recruiting for a Community Care Social Worker to join a supportive Pathway 3 service. You’ll play a vital role in helping people leave hospital safely, coordinating effective discharge plans and ensuring the right support is in place for patients and their families.

The role

As part of the Pathway 3 service, you’ll support safe and timely hospital discharge for people with more complex care needs. You’ll work closely with patients, families, health colleagues and community providers to:

  • Complete strengths-based assessments and care planning
  • Coordinate discharge arrangements and onward community support
  • Manage safeguarding and risk appropriately
  • Help prevent delayed discharges while ensuring safe outcomes
  • Work collaboratively across health and social care services

What you’ll need

  • A recognised Social Work qualification
  • Active Social Work England registration
  • Experience in hospital discharge, community care, adult social care or complex case management is highly desirable
  • The ability to manage a fast-paced caseload and undertake community visits
