The role

  • Completing Care Act assessments, reviews and support plans
  • Managing complex caseloads within the community
  • Undertaking safeguarding enquiries and risk assessments
  • Working closely with families, health professionals, providers and partner agencies
  • Supporting adults to access the right care, services and resources
  • Promoting independence, choice and positive outcomes

What you’ll need

  • A recognised Social Work qualification
  • Active Social Work England registration
  • Experience in Adult Social Care, preferably within a locality or community team
  • Confidence managing a busy caseload and undertaking community visits
  • Strong assessment, safeguarding and care-planning skills
