As a Nurse Case Manager RGN, you’ll coordinate complex-care packages for adults, using your clinical expertise to assess, plan, and manage person-centred care in home and community settings. This hybrid position offers regional travel, autonomy, and genuine career progression.
Key Responsibilities- Manage a caseload of community complex-care clients across the Leicestershire region, ensuring safe and effective delivery.
- Carry out clinical assessments, risk reviews, and care-plan updates in line with best practice and CQC standards.
- Provide clinical supervision, mentorship, and competency sign-offs for carers and support staff.
- Conduct clinical audits, medication checks, and quality reviews to maintain compliance and governance.
- Liaise with families, ICBs, case managers, and MDT professionals to coordinate holistic care.
- Deliver training and clinical support to teams during both routine and emergency situations.
- Drive continuous improvement and uphold professional nursing standards as a Nurse Case Manager RGN.
- Salary £40,000 – £45,000 per annum (DOE)
- Mileage reimbursed at 45p per mile
- Hybrid / flexible Monday–Friday working
- 25 days holiday + bank holidays + birthday off
