Responsibilities:
- Support statutory Care Act Assessments for Older People and clients with Physical Disabilities.
- Meet 28-day performance targets and reduce waiting times for new referrals.
- Handle complex long-term cases, addressing issues such as hoarding, non-engagers, DoLS challenges, and court cases related to welfare disputes.
- Conduct six-week reviews, performing Mental Capacity Assessments, completing Continuing Healthcare assessments, and advancing Safeguarding cases exclusively assigned to Social Workers within the team.
Qualifications:
- Social Work Qualification and registration with SWE.
