Key Responsibilities Support medicines optimisation through medication review, reconciliation and prescription review. Provide expert medicines advice to patients, carers, consultants and staff. Assist with safe procurement, management and control of medication stock. Contribute to clinical governance, audits, incident reporting and the introduction of new medicines and therapies.About You You are a registered Pharmacist with the GPhC. You have hospital pharmacy experience and a clinical diploma or equivalent experience.
