Leaders in Care are recruiting for a Social Work role within a busy Transfer of Care / Hospital Discharge Team.

This permanent hybrid position focuses on helping people return home safely and regain independence.

Based in a fast-paced hospital environment, you will complete assessments within a 14–28 day timeframe and conduct 6–12 week reviews, collaborating with health professionals and partner agencies to deliver effective discharge planning and reduce avoidable hospital readmissions.
