You’ll be joining a supportive multidisciplinary team, working collaboratively with colleagues across Adult Social Care, health, housing and community services to achieve positive outcomes for adults with complex needs.

  • Locum contract with the opportunity to contribute to a new and developing service.
  • Weekly check-ins and ongoing support throughout your placement.

Key Responsibilities

  • Complete strengths-based assessments for adults with complex needs.
  • Develop person-centred support plans that promote independence and positive outcomes.
  • Manage safeguarding concerns, risk assessments and complex casework.
  • Apply the Care Act, Mental Capacity Act and relevant safeguarding legislation in day-to-day practice.
  • Work with adults experiencing mental health needs, learning disabilities, autism and other complex circumstances.
  • Work closely with health, housing, voluntary-sector and community partners.

Requirements

To be considered for this position, you must:

  • Hold a recognised Social Work qualification.
  • Have current Social Work England registration.
  • Have relevant experience within Adult Social Care.
  • Possess sound knowledge of the Care Act 2014, Mental Capacity Act 2005 and adult safeguarding procedures.
