About the Service
You will be joining a dedicated Children with Disabilities Team committed to improving outcomes for children and young people with a range of complex needs and disabilities. The service places a strong emphasis on child‑centred practice, partnership working with families, and ensuring children have access to the support and resources they need to thrive.
Key Responsibilities
- Manage a caseload of children and young people with disabilities and complex needs
- Undertake Child and Family Assessments and statutory assessments
- Complete Care Act and Short Break assessments where appropriate
- Develop, implement, and review child‑centred care and support plans
- Work closely with families, schools, health professionals, and partner agencies
- Assess and manage safeguarding concerns in line with statutory guidance
- Support children in need, child protection, and looked after children processes where required
- Prepare high‑quality reports, assessments, and court documentation when necessary
- Ensure the voices of children and young people remain central to all planning and decision‑making
About You
- Qualified Social Worker
- Registered with Social Work England
- Recent experience within a Children with Disabilities, Safeguarding, Child in Need, or Assessment Team
- Strong knowledge of legislation relating to children with disabilities and safeguarding
- Experience working with children and young people with complex needs
- Excellent assessment, care planning, and relationship‑based practice skills
- Ability to build effective partnerships with families and multi‑agency professionals
