Responsibilities
- Ensure safe and smooth discharge of patients from the hospital.
- Collaborate with patients, families, and healthcare professionals to coordinate post‑discharge services.
- Develop and implement appropriate care plans tailored to individual patient needs.
- Act as a liaison between hospital staff and external care providers to ensure continuity of care.
- Address any challenges or concerns to facilitate a seamless transition for patients.
Qualifications
To be considered for this position, it is essential to hold a Social Work Qualification and maintain registration with SWE.
