We’re recruiting experienced Rapid Discharge Social Workers to join a busy and supportive Adult Social Care team, helping individuals leave hospital safely and efficiently while ensuring the right support is in place.

As a Rapid Discharge Social Worker, you’ll play a key role in supporting urgent and timely discharges from hospital settings, ensuring individuals receive the right care and support to return home safely. Based within the hospital environment, you’ll work closely with NHS colleagues, ward teams, and community services to assess needs quickly, arrange appropriate support, and reduce delays in discharge.

You’ll be part of a 7-day service, working a 35-hour week and completing at least one weekend day (Saturday or Sunday) as part of the rota.

To be considered, you’ll need:
  • A recognised Social Work qualification (BA, MA, DipSW or equivalent).
  • Current Social Work England registration.
  • At least 2–3 years’ post-qualified experience within Adult Social Care.
  • Previous experience within Hospital Discharge, Rapid Response, or Discharge to Assess services.
  • Strong knowledge of the Care Act, safeguarding procedures, and strengths-based practice.
  • The ability to work confidently in a fast-paced hospital environment.
