We’re offering an exciting opportunity within a fast-paced Transfer of Care / Hospital Discharge Team, where your work has an immediate and meaningful impact. At Leaders in Care, we understand the commitment and challenges that come with a career in social work. That’s why we work with the onboarding teams to help give you a smooth transition into a stable permanent role.
This role also comes with great benefits - including hybrid working. Enjoy up to two days a week from home, giving you the flexibility to balance work with your personal and family life. You’ll be working in a dynamic hospital setting supporting timely and safe discharges. The service operates across a 200-bed pathway, where individuals are supported to return home with packages of care, with the option for re-referral if ongoing support is required.
Your role will involve completing assessments within 14–28 days, undertaking 6–12 week reviews, and contributing to a light-touch triage system that ensures people receive the right level of support at the right time. You’ll play a key role in ensuring smooth transitions from hospital to home, promoting independence and reducing readmissions.
You’ll be joining a collaborative and supportive multidisciplinary team, working closely with healthcare professionals to deliver person-centred discharge planning and ongoing care. Expect a positive team culture where your voice is heard, your development is supported, and your wellbeing is prioritised.
As this is a statutory role, you must be a qualified Social Worker and registered with Social Work England.
