As Nurse Case Manager, you will oversee and manage complex care packages for adults across the Merseyside region, ensuring safe, high-quality care is delivered in the community.
Key responsibilities include:
- Managing and reviewing complex care packages
- Completing client assessments and developing tailored care plans
- Providing clinical guidance and support to care teams
