I'm recruiting on behalf of a respected complex care provider for a Paediatric Nurse Case Manager to join their specialist team on a full-time, permanent basis. This is a varied and rewarding role where you'll take responsibility for a caseload of children with complex care needs, providing clinical oversight, supporting care delivery and working closely with families and healthcare professionals to ensure high-quality, person-centred care.

Day to Day:
You'll be responsible for providing clinical oversight across a caseload of children with complex needs. Day to day, you'll:
- Manage and review a caseload of children in the community
- Complete assessments and care reviews
- Oversee risk assessments, care plans and clinical documentation
- Supervise and support care staff
- Carry out competency sign-off
- Liaise with families, GPs, consultants, community teams and commissioners
- Support new package setups and staff inductions
- Respond to clinical escalations
- Contribute to audits, quality assurance and CQC compliance
What We're Looking For:
- Registered Children's Nurse with active NMC PIN
- Strong experience in tracheostomy and ventilation care
- Experience in community nursing, complex care, hospital paediatrics or clinical leadership
- A calm, confident communicator who builds strong relationships easily
- Full UK driving licence and access to a vehicle
- Based along the M62 corridor
- Organised, proactive and comfortable working independently
