The role As an Adult Social Worker, you will support adults across a range of community settings, including:
Private homes Supported-living services Residential homes Nursing homes
You will manage a varied caseload and undertake meaningful, statutory Adult Social Care work, including:
Safeguarding enquiries and risk management Mental Capacity Assessments and Best Interest decisions Court of Protection work Assessing, implementing and reviewing care and support packages Managing referrals for supported living, residential and nursing placements Working closely with adults, families, carers, health professionals, providers and partner agencies Delivering strengths-based assessments, care planning and case management focused on independence and positive outcomes
What we're looking for To be considered, you will need:
A recognised
Social Work qualification Current
Social Work England
registration Previous experience within Adult Social Care Strong working knowledge of adult safeguarding, the Mental Capacity Act and Best Interest processes Confident assessment, care planning and case-management skills A person-centred approach and commitment to helping adults achieve the outcomes that matter to them The ability to manage competing priorities while maintaining high-quality case recording and practice
