We are recruiting for a Clinical Case Manager RMN to join a specialist community complex care provider delivering high quality mental health support packages across London and surrounding areas. This role is ideal for an experienced Registered Mental Health Nurse with a strong background in community-based care, case management, clinical governance and supporting individuals with complex mental health needs within their own homes or supported living environments.
As a Clinical Case Manager RMN, you will take ownership of complex packages, ensuring safe delivery, strong risk management and person-centred outcomes across a varied caseload.

The Role
You will provide clinical oversight across community mental health packages, supporting service users with complex and often high-risk presentations. You will work closely with families, MDTs, commissioners and operational teams to ensure safe, outcome-focused care delivery across the community.
Key Responsibilities
- Clinical oversight of complex mental health community packages.
- Completion of assessments, reviews, care plans and risk assessments.
- Managing safeguarding concerns and escalation processes.
- Supporting staff competency, supervision and clinical guidance.
- Liaising with psychiatrists, community teams, case managers and local authorities.
- Supporting crisis management and risk reduction strategies.
- Ensuring compliance with CQC and governance frameworks.
- Maintaining accurate clinical documentation and audit standards.
- Building strong relationships with families and external stakeholders.
Requirements
- Active NMC registration as an RMN.
- Experience within community mental health, complex care or case management.
- Strong understanding of safeguarding, risk management and escalation.
- Experience supporting service users with complex mental health presentations.
- Confident working autonomously within community settings.
- Strong communication and stakeholder management skills.
- Full UK driving licence preferred.
