Looking to step up and take on more responsibility while still staying close to practice? This is a fantastic opportunity for an experienced Social Worker to join a Transfer of Care / Hospital Discharge Service as a Senior Practitioner, where you'll play a key role in both frontline delivery and team support.
You’ll also benefit from hybrid working, with the flexibility to work from home up to two days per week helping you maintain a healthy balance between work and life outside of it. As this is a permanent role, we understand how tedious it can be waiting for local authority onboarding processes to be finalised. That’s why we not only liaise on your behalf with hiring managers, but also work proactively to get you started in your new role as quickly as possible.
Within this role, you’ll support a busy service delivering safe and timely discharges across a 200-bed pathway. You’ll hold a reduced caseload of more complex work, while also supporting the wider team to ensure assessments and reviews are completed within expected timeframes. You’ll act as a key point of support for less experienced staff, offering guidance around decision-making, prioritisation, and best practice.
Working within a light-touch triage system, you’ll help to ensure cases are appropriately managed and that workflows remain efficient and person-centred. This is an ideal role for someone who enjoys mentoring others, maintaining high standards, and being a reliable source of knowledge within a fast-paced environment without stepping fully away from hands-on social work.
You’ll be working closely with a multidisciplinary network, contributing to effective discharge planning and ensuring individuals are supported to return home safely with the right care in place. You’ll join a service that values collaboration, structure, and supportive leadership where your experience and input will genuinely shape outcomes for both the team and the people you support.
As this is a statutory role, you must be a qualified Social Worker and registered with Social Work England, with strong experience within adult social care and confidence supporting others in practice.
