This opportunity would also suit a Complex Care Coordinator or Care Scheduling Coordinator looking for greater variety and long-term progression.

As a Care Coordinator, you'll work across office and community settings, supporting children and adults with complex healthcare needs. You'll be joining a busy and established service delivering more than 8,000 hours of care each week, with no on-call responsibilities during your first 12 weeks.

Key Responsibilities

  • Manage a caseload of complex care packages, ensuring safe, effective and person-centred service delivery.
  • Coordinate rotas, staffing and contingency plans to maintain continuity of care and exceptional outcomes for service users.
  • Recruit, develop and support care teams through inductions, supervisions, appraisals and ongoing performance management.
  • Build strong relationships with families, commissioners and healthcare professionals, representing the service at reviews, MDT meetings and within the local community.

About You

  • Previous experience as a Care Coordinator, Complex Care Coordinator or similar role within care at home services.
  • Experience coordinating complex care packages and managing rotas within a healthcare setting.
  • Full UK driving licence and access to your own vehicle.
