Job Description

Community Nurse Case Manager Complex Care
Location: Chesterfield & surrounding areas
Salary: Circa £40,000 (negotiable depending on experience)
Full-time | MondayFriday 9am5pm (flexibility required)
We are working with a well-established Complex Care provider who has recently acquired a new service in Chesterfield and is now looking to grow and develop their Complex Care division in the area. As part of this exciting expansion, they are seeking a dedicated and passionate RGN-qualified Nurse Case Manager to play a key role in shaping and developing services locally.
At present, the service delivers around 3,000 hours of homecare, and this role will be mainly office-based initially, supporting service development, clinical governance, and workforce readiness while new complex care packages are grown and established in the community.
The Role
As a Community Nurse Case Manager, you will manage a caseload of clients with complex needs, supporting them to live safely and independently at home and within their wider communities. The role is varied and includes:
  • Completing initial assessments in the community
  • Training, assessing, and supporting care staff competencies
  • Allocating and overseeing support workers for individual packages
  • Working collaboratively with the wider MDT
  • Attending meetings and making onward referrals
  • Holding team meetings and providing clinical leadership
  • Managing and supporting a team of non-clinical Case Managers

About You
We are looking for an RGN-qualified Nurse with strong clinical experience in complex care, including:

  • Tracheostomy management (essential)
  • PEG feeding
  • Ventilation support

You will be motivated, organised, and passionate about developing high-quality complex care services, with the confidence to help grow a service from its early stages.
Additional Information

  • All services are rated Good by CQC, with some elements of Outstanding
  • You must be a car driver due to community elements of the role
  • Salary is negotiable, circa £40,000, depending on experience
