Your responsibilities will include:
Completing timely, strengths-basedCare Act assessmentsSupporting discharge planning from acute hospital and community settingsWorking closely with multidisciplinary teams to coordinate safe discharge arrangementsCompletingMental Capacity Assessmentsand contributing to Best Interest decisions where requiredArranging care packages, reablement and community-based supportPromoting independence and helping to prevent avoidable readmissionsLiaising with individuals, families, advocates and external professionals throughout the discharge processTo be considered, you will need: A recognisedSocial Work qualificationCurrentSocial Work England registrationExperience inHospital Discharge, Discharge to Assess, Adult Social Care or Adult Community ServicesStrong knowledge of theCare Act,Mental Capacity Actand strengths-based practiceConfidence managing complex cases in a busy multidisciplinary settingExperience using Mosaic, or another case management system, would be beneficialIf youre looking for a meaningful role where you can make a genuine difference to adults and families at a crucial point in their care journey, wed love to hear from you.
