Youll be joining a supportive multidisciplinary team, working collaboratively with colleagues across Adult Social Care, health, housing and community services to achieve positive outcomes for adults with complex needs.

Whats on Offer? Flexible/Hybrid Working Competitive hourly rate:

up to

Β£40 per hour . 36 hours per week. Locum contract with the opportunity to contribute to a new and developing service. Weekly check-ins and ongoing support throughout your placement.

Key Responsibilities Complete strengths-based assessments for adults with complex needs. Develop person-centred support plans that promote independence and positive outcomes. Manage safeguarding concerns, risk assessments and complex casework. Apply the Care Act, Mental Capacity Act and relevant safeguarding legislation in day-to-day practice. Work with adults experiencing mental health needs, learning disabilities, autism and other complex circumstances. Work closely with health, housing, voluntary-sector and community partners.

Requirements

To be considered for this position, you must: Hold a recognised

Social Work qualification . Have current

Social Work England registration . Have relevant experience within

Adult Social Care . Possess sound knowledge of the

Care Act 2014 ,

Mental Capacity Act 2005

and adult safeguarding procedures.
