Your main duties will include:
- Supporting and advocating for adults with complex health and care needs through the Continuing Healthcare (CHC) pathway.
- Working collaboratively with multidisciplinary professionals, service users, and families to assess needs and develop effective care plans.
- Contributing to CHC assessments, reviews, and decision-making processes to ensure individuals receive appropriate funding and support.
- Promoting independence, dignity, and person-centred care for adults with long-term health conditions and complex needs.
