As a Community Nurse, you will:
- Cover the Leeds / West Yorkshire region, travelling to patients' homes.
- Work as part of a multi-disciplinary team offering one-to-one care.
- Deliver effective and high-quality nursing care to facilitate early supported transfer from hospital or prevent unnecessary admission.
- Build lasting relationships with patients, stakeholders, and patient support networks.
- Accurately document all patient care and clinical observations.
