Are you an experienced community nurse looking for a Community Clinical Lead Nurse role that offers genuine clinical autonomy, a manageable caseload, and a strong work-life balance? This Community Clinical Lead Nurse position is based in London and involves overseeing a small, mixed caseload of adult and paediatric complex care packages across North West and West London. You will have protected time for assessments, care planning, and clinical oversight, allowing you to focus on quality, safety, and outcomes.
You will be joining one of the UK’s leading complex care providers, known for delivering high-quality, nurse-led services across both adult and paediatric pathways.
The role
- Act as the named clinical lead for a defined caseload of community complex care packages
- Complete assessments, reviews, and ongoing clinical monitoring
- Develop and review bespoke, person-centred care plans
- Provide clinical oversight, supervision, and staff support
- Deliver clinical training and competency assessments
- Work collaboratively with MDTs, CHC nurses, commissioners, schools, and therapy teams
Requirements
- Registered Nurse with an active NMC PIN
- Adult, Children’s, or dual-qualified nurses welcome
- Minimum of 18 months UK nursing experience with recent complex care exposure
- Confidence working across adult and paediatric care packages, or willingness to develop
- Band 5 or Band 6 nurses with leadership or senior shift experience encouraged to apply
