A leading national provider of complex care services is looking for an experienced Nurse Case Manager (RGN) to manage and oversee complex care packages for adults living at home across the West Yorkshire region.

As Nurse Case Manager, you will oversee and manage complex care packages for adults across the Merseyside region, ensuring safe, high-quality care is delivered in the community.

Key responsibilities include:

  • Managing and reviewing complex care packages
  • Completing client assessments and developing tailored care plans
  • Providing clinical guidance and support to care teams
  • Working closely with families and healthcare professionals to coordinate care
  • Ensuring high standards of clinical governance and complex care delivery

To be considered for this role you must have:

  • Registered General Nurse (RGN) qualification
  • Active NMC PIN with no restrictions
  • Recent tracheostomy and ventilator experience (within the last 2 years)
  • Experience supporting or supervising care staff
  • Strong clinical assessment and care planning skills
  • Full UK driving licence
