The Role
As Nurse Case Manager, you'll take ownership of complex care packages for adult clients, ensuring safe, high-quality delivery while developing and supporting care teams.
Key responsibilities:
- Managing and reviewing complex care packages in the community
- Leading clinical quality, governance, and best practice
- Delivering specialist training (e.g. tracheostomy, ventilation)
- Assessing and signing off staff competencies
- Coaching and mentoring care teams
- Building strong relationships with clients, families, and MDTs
About You
- Registered Nurse with NMC PIN
- Experience in complex care (e.g. tracheostomy, ventilation, spinal injury)
- Passion for developing others and improving care standards
- Strong communication and organisational skills
- Confident working independently in a community setting
- Full UK driving licence and access to a vehicle
