This is a fantastic opportunity for an experienced Social Worker or Team Manager to step into a pivotal leadership role within a Hospital Discharge Service. You'll also benefit from hybrid working, with the flexibility to work from home up to two days per week, helping you maintain a healthy balance between leadership responsibilities and life outside of work.

In this role, you'll oversee a busy service supporting safe and timely discharges across a 200-bed pathway. You'll play a key role in allocating cases, monitoring performance, and ensuring that assessments and reviews are completed within expected timeframes including 14-day assessments and 6-week reviews. This is a hands-on leadership role where you'll guide and support your team through a light-touch triage system, ensuring workflows are efficient and outcomes remain person-centred.

You'll be instrumental in maintaining high standards of practice while supporting staff with decision-making, prioritisation, and complex cases. You'll be working alongside a strong multidisciplinary network, helping to drive effective discharge planning and ensuring individuals are supported to return home safely with the right level of care in place.
