This Hospital Discharge team supports adults to leave hospital safely and get home with the right care in place - no unnecessary complexity, just good social work.

What youll be doing:
Youll be supporting safe hospital discharges, completing and overseeing Care Act assessments, and putting practical care plans in place to help prevent re-admissions. Alongside your own casework, youll supervise a small team and work closely with health colleagues to keep discharges moving and make sure people dont get stuck in hospital unnecessarily.
Youll need:
- Social work qualification
- Registration with Social Care Wales
- 3 years post qualifying experience preferred
