The role
As an
Adult Community Social Worker , you will work with adults aged 18+ to promote independence, achieve positive outcomes and ensure the right care and support is in place.
Your responsibilities will include:
Managing a community-based caseload of adults with complex needs Undertaking mental capacity and best-interest assessments where required Supporting adults, families and carers to identify the outcomes that matter most to them Working closely with health professionals, care providers and voluntary-sector partners Safeguarding adults at risk and contributing to timely hospital discharges Maintaining accurate case records and delivering person-centred, legally literate practice
