We are looking for a Care Coordinator to join an established homecare provider in Bournemouth. You'll work Monday to Friday, 9am–5pm, alongside additional on-call duties on a rota basis.

As a Care Coordinator, you'll be responsible for:

Coordinating care rotas and ensuring all visits are effectively covered
Supporting and supervising care workers in the community
Assessing and reviewing individual care plans
Completing field-based visits with clients and care staff
Building strong relationships with clients, families and care workers
Responding to changes in client needs and resolving day-to-day issues
Ensuring care is delivered safely and to a high standardYou'll need previous experience within care coordination or a similar community care role, with strong organisational and communication skills and the ability to manage busy rotas effectively.

A full UK driving licence and access to your own vehicle are essential. xohmjla
