The Opportunity

We are working with an established and growing complex care provider that is looking to expand its team of Care Coordinators across the North West. This is a fantastic opportunity to join an established organisation with a strong team of Healthcare Assistants and Nurses, supporting the continued growth of its complex care services across the region. We are particularly interested in candidates based in or around Rochdale, Burnley and Tameside, although applicants from surrounding areas will also be considered.

The Role

As a Care Coordinator, you will play an important role in ensuring high-quality care is delivered to clients while supporting and coordinating the HCA and nursing teams. Your responsibilities will include:

  • Coordinating care packages and ensuring appropriate staffing levels are maintained.
  • Managing rotas and ensuring shifts are appropriately covered.
  • Supporting the recruitment, onboarding and allocation of HCAs and Nurses.
  • Building strong relationships with clients, families and healthcare professionals.
  • Carrying out regular staff and client check-ins.
  • Supporting care plans and ensuring they remain accurate and up to date.
  • Monitoring the quality of care being delivered and escalating concerns appropriately.
  • Working closely with the wider operational and clinical teams.
  • Supporting the growth and development of new and existing care packages.
  • Ensuring care is delivered in line with company policies, procedures and regulatory requirements.

About You

We are looking for someone who is organised, proactive and confident working in a fast-paced care environment. You will ideally have:

  • Previous experience within care coordination, care management, domiciliary care or a similar role.
  • Experience within complex care or community-based care would be advantageous.
  • A good understanding of care planning, rotas and coordinating care teams.
  • Excellent communication and relationship-building skills.
  • The ability to manage competing priorities and work independently.
  • A genuine passion for delivering high-quality, person-centred care.
  • Confidence working with HCAs, Nurses, clients, families and external professionals.
