I'm recruiting on behalf of a highly respected, established and financially stable complex care provider for a Paediatric Complex Care Nurse Case Manager to join their specialist clinical team. This is a rewarding community-based role where you'll manage your own caseload of children with complex healthcare needs, providing clinical leadership and ensuring outstanding, person-centred care is delivered to children and their families across North East London. The role covers predominantly the N, E and EN postcode areas, therefore applicants should ideally be based within or close to these locations.

The Role
As a Paediatric Complex Care Nurse Case Manager, you'll provide clinical oversight and leadership across a caseload of children receiving care within their own homes. Key responsibilities include:
- Managing and reviewing complex community care packages
- Completing comprehensive clinical assessments and regular care reviews
- Developing and reviewing care plans, risk assessments and clinical documentation
- Providing clinical supervision, support and guidance to care teams
- Undertaking competency assessments and clinical sign-off for care staff
- Supporting the mobilisation of new care packages
- Delivering staff inductions, training and ongoing clinical support
What We’re Looking For
- Registered Children's Nurse (RSCN) with an active NMC PIN
- Strong experience caring for children with complex healthcare needs
- Excellent clinical skills in areas such as tracheostomy care, long-term ventilation, enteral feeding, PEG care, seizure management and complex respiratory care
- Experience within community nursing, complex care, hospital paediatrics, case management or a senior clinical leadership role
- Excellent communication and relationship-building skills
- Organised, proactive and comfortable working autonomously
- Full UK driving licence and access to a vehicle
