Responsibilities:
- Ensure safe and smooth discharge of patients from the hospital.
- Collaborate with patients, families, and healthcare professionals to coordinate post-discharge services.
- Develop and implement care plans tailored to individual patient needs.
- Act as a liaison between hospital staff and external care providers to ensure continuity of care.
- Address challenges to facilitate seamless patient transitions.
