As Nurse Case Manager, you will oversee and manage complex care packages for adults across the Merseyside region, ensuring safe, high‑quality care is delivered in the community.
Key responsibilities:
- Managing and reviewing complex care packages
- Completing client assessments and developing tailored care plans
- Providing clinical guidance and support to care teams
- Working closely with families and healthcare professionals to coordinate care

To be considered for this role you must have:
- Registered General Nurse (RGN) qualification
