The Role

We're recruiting experienced Discharge to Assess Social Workers to join a busy and supportive team focused on delivering safe, timely hospital discharges.

Working closely with health professionals, therapists, ward staff, and community partners, you'll complete strengths-based assessments following discharge, coordinate care and support, and ensure individuals receive the right services to maximise independence and reduce unnecessary readmissions.

You’ll be responsible for:

  • Completing Care Act assessments within the Discharge to Assess pathway.
  • Supporting safe and timely discharges from acute hospital settings.
  • Undertaking Mental Capacity Assessments and Best Interest decisions where required.
  • Coordinating care packages and community-based support.

About You

To be considered, you'll need:

  • A recognised Social Work qualification.
  • Current Social Work England registration.
  • Experience working within Hospital Discharge, Discharge to Assess, or Adult Community Services.
  • A sound understanding of the Care Act, Mental Capacity Act, and strengths-based practice.
  • Experience using Mosaic or a similar case management system is desirable.
