About the Role

You can work from home two days a week, giving you the flexibility to balance your professional and personal life. Responsibilities

  • Work within a multidisciplinary team, conducting assessments, care planning, and interventions to support adults with complex needs.
  • Promote independence, arrange tailored support, and ensure individuals receive the right care to enhance their quality of life.

Qualifications & Experience

  • Job‑ready Social Work Qualification and registration with Social Work England.
  • At least 3 years of post‑qualification experience.
  • MUST have experience with Court Of Protection, Safeguarding and working with Mosaic.
