About the Role

Responsibilities
- Work within a multidisciplinary team, conducting assessments, care planning, and interventions to support adults with complex needs.
- Promote independence, arrange tailored support, and ensure individuals receive the right care to enhance their quality of life.
Qualifications & Experience
- Job‑ready Social Work Qualification and registration with Social Work England.
- At least 3 years of post‑qualification experience.
- MUST have experience with Court Of Protection, Safeguarding and working with Mosaic.
