Main duties of the job
We are seeking a proactive GP Assistant (GPA) to support the delivery of diabetes care within our practice. The post holder will play a key role in identifying patients due for diabetes reviews, coordinating follow‑up care, undertaking routine clinical assessments and supporting patients to manage their condition. Working as part of the multidisciplinary team, they will contribute to the delivery of high‑quality, patient‑centred diabetes services through health promotion, patient education and care coordination.
Job responsibilities:
- Proactively identify patients with diabetes who are due or overdue for annual reviews, monitoring, screening and follow‑up appointments.
- Support the recall and care‑coordination process for patients with diabetes, ensuring appointments, investigations, results and reviews are completed and appropriately documented.
- Order and coordinate required tests in accordance with Practice protocols and under appropriate clinical supervision.
- Provide a phlebotomy service and undertake routine clinical assessments including blood pressure monitoring, height, weight, BMI measurements, urinalysis and diabetic foot checks.
- Support the completion of annual diabetes reviews by ensuring all required checks, investigations and information are obtained, recorded and coded accurately within the clinical system.
- Use patient contacts as opportunities to provide health promotion, education and support around diabetes management, healthy lifestyles, weight management, smoking cessation and physical activity.
- Promote patient self‑management and support referrals to structured diabetes education programmes, such as DESMOND, and other appropriate community services.
- Identify current and future health and care needs and signpost or refer patients to appropriate services in line with Practice protocols.
- Ensure patients who do not attend appointments are followed up in accordance with Practice procedures.
- Work collaboratively with GPs, nurses, healthcare assistants, pharmacists and other members of the multidisciplinary team to support the delivery of high‑quality diabetes care.
- Act at all times with regard to the confidentiality and sensitivity of issues relating to patients, their care and treatment.
- Participate in audits, quality improvement initiatives and service development activities relating to diabetes and long‑term condition management.
Experience:
- Experience of working in a healthcare setting
- Experience of working with the general public
- Experience of working in a primary care environment
Clinical Knowledge:
- ECGs
- New patient medicals, including height, weight, BP, pulse
- Ability to record accurate clinical notes
Qualifications:
- Healthcare qualification (level 3 or 4) or working towards gaining equivalent level
- GP Assistant Certificate qualification or working towards the qualification
Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions. Depending on experience, equivalent to AFC Band 3 or 4.
