The predicted salary is between 48000 - 72000 ÂŁ per year.

The role of the Digital Clinical Lead will support continuous improvement within the digital delivery of the surgeries and will be an integral part of the core multidisciplinary primary care team within the Hammersmith & Fulham Health Centres. The role will also require clinical sessions to be undertaken at the practice.

Core Hours: 6 sessions per week (split between GP Lead sessions & Clinical Sessions)

Main duties of the job

The post holder will lead and provide the full range of Alternate Provider Medical Services in collaboration with other healthcare professionals within the practice. They will also lead and deliver on national quality markers such as the Quality and Outcomes Framework (QOF), Enhanced Services, and other national and local targets as determined by PCN/ICB provision, through robust quality assurance systems, appropriate use of skill‑mix, use of information technology and by education and development of our staff.

The post holder will be a motivated and experienced GP with a range of digital and clinical experience and skills. The clinical lead will be expected to work clinical sessions at the practice to ensure we continue to deliver high quality and innovative care and ensure that this new strategic leadership role is driving cultural change, cost efficiency and service redesign. The post holder will also be the clinical point of contact for the West London NHT team in hours and oversee matters relating to LCW clinical contacts at the care homes.

LCW also partner with West London Mental Health Trust to support with their enhanced services for patients residing in their care homes between 6pm and 8pm Monday to Friday and 8am to 8pm Saturday and Sunday and bank holidays.

Job responsibilities

Key Responsibilities

  • Digital Clinical Leadership
  • To be the Clinical Safety Officer (CSO) for the practice ensuring compliance with NHS Digital safety standards prior to deployment of all new clinical digital systems.
  • Lead the transformation & implementation of the service including (but not limited to) the digital front door, management of long term conditions, delivery of key performance such as QOF.
  • Lead the clinical design and continuous improvement of digital workflows e.g. online triage systems, appointment pathways, digital messaging, remote monitoring, data analytics.
  • Ensure digital innovation improves patient access, clinical safety, workflow efficiency and care quality using population health data.
  • Work with the Practice Manager to ensure all digital clinical systems are used safely and optimally.
  • Conduct regular audits of digital consultation quality, patient outcomes and system performance.
  • Lead on training for staff in new digital systems.
  • Support cybersecurity awareness and safe digital practice across the clinical team.
  • Be the key digital clinical liaison representative for the practice at PCN and ICB level.
  • Success will demonstrate improved access metrics, reduced DNAs, enhanced patient experiences, more effective use of skill mix, improved delivery of KPIs related to APMS contracts.
  • Daily GP Responsibilities
    • Provide a full range of medical services as defined in the APMS contract, additional and enhanced services where appropriate.
    • Make professional, autonomous decisions in relation to presenting problems whether self‑referred or referred from other healthcare professionals within the organisation.
    • Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
    • Screen patients for disease risk factors and early signs of illness.
    • Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education.
    • Maximise the effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to an agreed standard.
    • Compile and issue computer generated acute and repeat prescriptions (avoiding handwritten prescriptions whenever possible).
    • Take part in areas including clinical auditing and data collection.
    • Contribute to QOF and Northwest London Standard requirements and recording & code data appropriately.
    • Attend team meetings and other clinical meetings as necessary.
    • In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working with primary care, as set by Royal College of General Practitioners, General Medical Council, all other Royal Colleges and Councils applicable to the staff within the Practice, Department of Health, ICB, CQC, and all other regulatory and standard setting organisations.
    • Work as part of the multi‑disciplinary clinical team to support management and target delivery in patients with long term conditions.
    • Work flexibly between sites, to provide the best continuity of care for patients.
    • Work in partnership with patients to provide high quality patient centred care.
    • Day to day responsibility for services commissioned by WL NHT.
  • Quality & Compliance
    • Be responsible for maintaining professional development. Attendance at clinical meetings, staff meetings and multidisciplinary meetings contribute towards CPD. Protected CPD time will be agreed upon appointment.
    • Work within current ICB policy for prescribing.
    • Take part and contribute to clinical auditing and data collection.
    • Attend and present at monthly LCW Quality Assurance Group.
    • Provide support to the quality, safety and experience team on quality and governance for services commissioned by WL NHT.
  • Stakeholder Engagement & Service Development
    • Key member in the PCN network.
    • Respond to stakeholder enquiries and ensure satisfactory resolution.
    • Support service improvement initiatives and change management projects.
    • Assist in investigating complaints, feedback, and patient safety incidents (PSIRF), ensuring timely action and shared learning.
    • Support LCW in its work to understand and continually improve patient experience through gaining feedback and taking actions to address.
    • Liaise with the Clinical Lead at the West London NHT should matters arise and attend quarterly meetings with the team to discuss service.
    • Identify and mitigate risks, ensuring they are logged in LCW's quality management system.

    Experience

    • Experience of working in primary care in an area with significant health problems.
    • Experience of providing primary care in and out of hours.
    • Experience working as a GP minimum 2 years.
    • Development of areas of clinical expertise.
    • Experience of audit and improving care for patients.
    • Be a trained CSO or willing to undertake training.
    • Experience in understanding and delivering Primary Care targets and KPIs.
    • Desirable if held lead/digital roles before.
    • Desirable experience in transformation and cultural change.

    Qualifications

    • MBBS or equivalent medical degree.
    • Full registration with GMC.
    • Certification of Completion of Training (JCPTGP/PMETB).
    • Inclusion on Performers List.
    • Evidence of a commitment to ongoing personal & professional development.
    • MRCGP.
